FAA CLL/SLL Status Summary for Pilots

Pilots with chronic lymphocytic leukemia or small lymphocytic lymphoma ask the oncologist who manages it to complete this one-page FAA summary. It records whether the diagnosis came at age 40 or older with 5 or more years of oncology follow-up, current stability, Rai and Binet stage, signs of active disease, and labs from the last 90 days. It goes with a current progress note.

Who completes it

The treating oncologist completes and signs the form and adds a name or office stamp. The pilot fills in name, birthdate, applicant ID and PI number.

Submitting and payment

How to submit it
The summary, a current, detailed Clinical Progress Note addressing each item, and a copy of the lab results go to the pilot's AME or are mailed to the FAA's Aerospace Medical Certification Division.

Good to know

  • Labs within 90 days must report hemoglobin, show platelets above 100,000/microL, and show a lymphocyte count doubling time longer than 6 months; the lab report is attached.
  • Examples of active disease include unintentional weight loss of more than 10 pounds and new palpable lymph nodes; the form suggests considering iwCLL criteria.
  • Item 2 asks whether the pilot is stable and asymptomatic with observation as the only recommended treatment.
  • Item 7 asks the oncologist to explain any NO answers or other concerns in the attached progress note. The CACI CLL/SLL Worksheet is the companion document.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
