FAA CHD/CAD Recertification Status Summary

Pilots who already hold a special issuance or special consideration for coronary heart disease have their cardiologist complete this one-page summary to speed recertification. The cardiologist records blood pressure, LDL-C and A1c from testing within 90 days of the AME exam, lists current therapy, and answers six yes or no questions on guideline-directed therapy, symptoms, functional capacity and interval events.

Who completes it

The treating cardiologist completes, signs and dates the form. The pilot fills in name, birthdate, applicant ID and PI number, and the AME acts on the answers.

Submitting and payment

How to submit it
The pilot returns the completed summary to the AME for electronic upload to the FAA. Any NO* answer or interval change requires an attached current, detailed Clinical Progress Note.

Good to know

  • A1c must be reported even if the pilot has no diabetes diagnosis, along with blood pressure and LDL-C from testing within 90 days of the exam.
  • Question 5 covers the past year: no catheterization, CABG, PTCA or stent, MI, atherectomy, brachytherapy, bleeding requiring intervention, or nitrate use for any reason.
  • Guideline-directed therapy and risk-factor control are judged against the 2023 AHA/ACC chronic coronary disease guideline cited on the form.
  • Initial CHD/CAD certification is always deferred. Since 05/29/24, routine recertification cases need no follow-up stress test unless FAA review asks for one.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can use the FAA CHD/CAD recertification status summary?

Only pilots with a valid special issuance authorization or special consideration for CHD/CAD. Pilots without one stop and follow the Coronary Heart Disease disposition table, and the AME must defer an initial CHD/CAD certification.

Does the FAA still require a stress test for CHD recertification?

As of 05/29/24, a follow-up stress test is not required for routine cases. If FAA review determines one is needed, the pilot submits this status summary together with a current, detailed Clinical Progress Note from the cardiologist.

What happens if the cardiologist answers NO* on the CHD summary?

Any NO* answer, significant interval history change or concern means the AME must defer. The cardiologist attaches a current, detailed Clinical Progress Note addressing those items. If every answer is YES and nothing has changed, a pilot with an AASI is handled under the AASI instructions.
