FAA OSA Specification Sheet A: Information Request

When a pilot's medical application shows treatment or a previous assessment for obstructive sleep apnea, the FAA uses Sheet A to request documentation within 90 days. The simplest answer is a completed OSA Treated Status Report; otherwise the pilot sends all OSA assessment records and, if still treated, compliance, sleep study and progress note documentation.

Who completes it

The FAA issues the sheet to the pilot. The primary care physician or sleep specialist treating the OSA completes the OSA Treated Status Report, or supplies the assessment records, the latest sleep study interpretation and a current Detailed Clinical Progress Note.

Submitting and payment

How to submit it
The pilot gives the documents to the AME for upload into the FAA file under the pilot's PI, MID or APP ID reference number, for AMCD or the Regional Flight Surgeon, within 90 days.

Good to know

  • CPAP, BiPAP and APAP users include the cumulative annual device report; the target is use on at least 75% of sleep periods, averaging at least 6 hours per period.
  • A treated pilot sending records instead of the status report also needs a signed FAA Compliance with Treatment sheet or equivalent and the latest sleep study with its interpretive report.
  • The progress note must state that OSA treatment is still effective. Reports from dental or positional devices become required once devices with recording capability are available.
  • Sheet A was updated 05/27/2026. Specification Sheet B is the separate request for a pilot at risk for OSA who has not been assessed.

Frequently asked questions

What is the simplest way to answer FAA OSA Specification Sheet A?

Have the treating physician complete and sign the OSA Treated Status Report, which covers diagnosis, medication safety, treatment effectiveness and compliance on one page. Otherwise the pilot must assemble the assessment records, a Compliance with Treatment sheet, the latest sleep study and a current progress note.

What PAP compliance does the FAA expect under Sheet A?

The cumulative annual PAP device report should show use on at least 75% of sleep periods with an average of at least 6 hours per sleep period. The AME uploads the report with the other OSA documents into the pilot's FAA file.

How long does a pilot have to respond to OSA Specification Sheet A?

90 days. The documents go to the Aerospace Medical Certification Division or the Regional Flight Surgeon through the AME, who uploads them into the pilot's FAA file using the PI, MID or APP ID reference number.
