FAA CACI Psoriasis Worksheet for Pilots

Pilots with psoriasis can be certified in the AME's office through this Conditions AMEs Can Issue (CACI) checklist. The AME reviews the treating physician's or dermatologist's progress note, from a visit within 90 days of the exam, and confirms mild to moderate, stable disease with no systemic symptoms or limits to strength, function or range of motion, an accepted subtype, and medications on the FAA list.

Who completes it

The AME completes the checklist and marks the Block 60 outcome. The treating physician or dermatologist writes the current, detailed Clinical Progress Note the AME reviews.

Submitting and payment

How to submit it
A qualifying case is noted as 'CACI qualified Psoriasis' in Block 60 of FAA Form 8500-8, and the documents do not need to be submitted. Otherwise the AME defers and submits the supporting documents.

Good to know

  • Most subtypes qualify, including guttate and plaque psoriasis. Pustular and erythrodermic psoriasis are not acceptable, and psoriatic arthritis is handled under the FAA arthritis page.
  • Topical medication, azathioprine and methotrexate are accepted, and may be combined with one listed systemic agent, which needs a 2-week ground trial.
  • Post-dose observation: none for apremilast; 4 hours for adalimumab, certolizumab, etanercept, guselkumab, ixekizumab, risankizumab, secukinumab and ustekinumab; 24 hours for infliximab.
  • If co-morbid conditions are present, the AME checks each one against its own AME Guide page as well.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is Humira or Skyrizi allowed under the FAA psoriasis CACI?

Yes. Adalimumab (Humira or an FDA-approved biosimilar) and risankizumab (Skyrizi) are on the list, each after a 2-week ground trial and with a 4-hour post-dose observation time. Only one such agent may be used, though it can be combined with topical medication and methotrexate.

Which types of psoriasis don't qualify for the CACI?

Pustular and erythrodermic psoriasis are not acceptable. Most other subtypes, including plaque and guttate, qualify if severity is mild to moderate and the condition is stable. Psoriatic arthritis is reviewed under the FAA arthritis page instead of this worksheet.
