FAA Form 8500-8: Airman Medical Certificate Application

Every first-, second- or third-class FAA medical certificate under 14 CFR Part 67 starts with this application. The pilot reports medical history in MedXPress, which transmits it to the FAA for the AME to review at the exam. The AME then records the physical exam, findings and certification decision in AMCS, so there is no fillable public PDF.

Who completes it

The pilot completes Items 1 to 20 in MedXPress. The Aviation Medical Examiner completes the exam items 21 to 58 and the review items 59 to 64, including Block 60 comments and the AME declaration, in AMCS.

Submitting and payment

How to submit it
The AME transmits the completed application and exam to AMCD through AMCS within 14 days of completion, unless the FAA directs otherwise, then issues the certificate or defers.
Who pays
The FAA does not set AME examination fees; the pilot pays the examiner, and the FAA recommends the usual and customary local fee for similar services.

Good to know

  • Only FAA-designated AMEs, appointed under FAA Order 8000.95, may perform the exam and complete the examiner's items.
  • Block 60 records CACI status, such as 'CACI qualified', and other comments. Supporting documents like Forms 8500-7 and 8500-14, CACI worksheets and status summaries accompany it when needed.
  • A third-class certificate is valid for the rest of the month of issue plus 60 calendar months for pilots under 40 at the exam, or 24 months at 40 or older.
  • Item 18.v asks about alcohol- or drug-related driving arrests, convictions and administrative actions; a single event leads the AME to the Alcohol Event Status Report.

Frequently asked questions

Can any physician complete FAA Form 8500-8?

No. Only an FAA-designated Aviation Medical Examiner can perform the exam and complete the examiner's items in AMCS. A pilot who wants to fly under BasicMed instead can see any state-licensed physician, who completes FAA Form 8700-2 rather than this application.

Is there a printable PDF of FAA Form 8500-8?

Not for completing the application. The pilot fills in the history portion in MedXPress, which sends it to the FAA for the AME to review, and the AME completes and transmits the rest through AMCS. The AME may print a summary sheet for the pilot.

How long is an FAA medical certificate valid?

It depends on class and age at the exam. Beyond the month of issue, first-class privileges last 12 calendar months under age 40 or 6 months at 40 and over, second-class privileges 12 months, and third-class privileges 60 months under 40 or 24 months at 40 and over.
