FAA Weight Loss or Prediabetes Status Report

Pilots taking medication for weight loss or prediabetes, common with GLP-1 receptor agonists, bring this one-page report, updated 04/29/2026, from the prescriber to the AME. It records why the drug is taken, the last visit and medication change, A1C, side effects, hypoglycemia, insulin use and every current medication, so the AME can apply the FAA's CACI or deferral pathway.

Who completes it

The provider who prescribes the weight-loss or prediabetes medication completes and signs the whole report; the pilot adds name, birthdate and FAA identifiers.

Submitting and payment

How to submit it
The completed report goes back to the pilot, who brings it to the AME for upload into the FAA file.

Good to know

  • A1C is required only when the medication is for prediabetes and must be dated within 90 days of the AME exam; the prescriber enters n/a when it is for weight loss only.
  • For GLP-1 medications, the prescriber must verify there are no psychiatric side effects. Any yes answer needs an attached narrative, tests or other records.
  • Under the FAA disposition table, one non-insulin diabetes medication component can be handled by the AME through the CACI worksheet; two or more components mean deferral for possible special issuance.
  • If the pilot has been diagnosed with diabetes or takes any insulin, the FAA's separate diabetes or insulin-treated diabetes pathways apply instead.

Frequently asked questions

Does a pilot on a GLP-1 for weight loss need an A1C on the FAA status report?

Not if the medication is for weight loss only; the prescriber enters n/a. When it is for prediabetes, the A1C value and date are required and the test must be within 90 days of the AME exam. The prescriber must also confirm there are no psychiatric side effects.

Can a progress note replace the Weight Loss Management or Prediabetes Status Report?

Yes. The FAA accepts a current, detailed Clinical Progress Note from the treating physician covering the same information, including the indication for the medication, whether there is any history of diabetes and, where required, an A1C within 90 days of the AME exam.
