FAA HIV Specification Sheet for Pilots

Pilots with HIV who do not have an AIDS diagnosis may be considered for any class of medical certificate, and this one-page FAA specification lists what the treating physician must provide. It covers a current report and immune-focused history, viral load by PCR, CD4 and lymphocyte counts, CBC with differential, liver function tests, BUN and creatinine, and a cognitive function assessment.

Who completes it

A physician knowledgeable in the treatment of HIV-infected persons writes the status report and orders the tests; the cognitive assessment follows the FAA neuropsychology testing specifications. There is no form to fill in.

Submitting and payment

How to submit it
All items are mailed together in one package to the FAA's Aeromedical Certification Branch. Current studies should be submitted no later than 30 days from the test date.

Good to know

  • Antiretroviral medication must be FDA-approved and used under an acceptable drug therapy protocol for the pilot to be considered.
  • Pilots who have had an AIDS-defining illness need two viral load and two CD4 determinations, one month apart.
  • Follow-up neuropsychological evaluations are annual for first and second class and every other year for third class, following the FAA Neuropsychology Testing Specifications.
  • Pilots with a history of cytomegalovirus retinitis need an ophthalmology evaluation with visual fields initially and every 6 months. Separate surveillance specifications cover under and after 2 years.
