FAA HIMS Psychiatric & Neuropsych Evaluation Specs

Pilots with a substance use disorder, whether abuse or dependence, must be in satisfactory recovery before the FAA will certify them. This three-page specification, updated 01/29/2020, requires two separate evaluations, psychiatric and neuropsychological, to assess the disorder, the quality of recovery and any other psychiatric condition or neurocognitive deficit, and lists what each report must contain.

Who completes it

A psychiatrist board-certified by the ABPN or the American Board of Osteopathic Neurology and Psychiatry, who is also board certified in addiction psychiatry or trained in the HIMS program, writes the psychiatric report. A neuropsychologist on the FAA's provider list performs the testing and writes the neuropsychological report.

Submitting and payment

How to submit it
Both reports, with supporting records, computer score reports and a score summary sheet, go to the FAA's Aerospace Medical Certification Division with the HIMS AME's package.

Good to know

  • The airman should give both evaluators prior psychiatric and substance treatment records and the complete FAA medical file, requested on FAA Form 8065-2.
  • Each report needs a records review, a clinical interview covering substance use treatment and quality of recovery, a mental status exam and an integrated summary with an explicit diagnosis.
  • The psychiatrist's recommendations may cover treatment, medication, therapy, counseling, rehabilitation or monitoring, but must stay within the psychiatrist's area of expertise.
  • The core test battery is listed only in the FAA's secure testing portal, and raw test data may later be requested for review by an FAA consulting psychologist.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What qualifications must the psychiatrist have for an FAA substance abuse evaluation?

Board certification by the American Board of Psychiatry and Neurology or the American Board of Osteopathic Neurology and Psychiatry, plus either board certification in addiction psychiatry or training in the HIMS program. The FAA prefers HIMS-trained psychiatrists and warns that one without this background may limit the report's usefulness.

Does the neuropsychologist need to be on an FAA list?

Yes. For substance abuse and dependence evaluations, the neuropsychologist must be on the FAA Neuropsychologist List. The report must interpret a full neuropsychological and psychological battery, including the core battery from the FAA's secure portal, with pilot norms where available.

Are repeat evaluations required after special issuance?

Pilots found eligible for unrestricted certification need no further evaluations. Those found eligible for special issuance undergo periodic re-evaluations, and the special issuance authorization letter spells out which evaluations or tests are required.
