FAA Six-Minute Walk Test (6MWT) Result Sheet

When the FAA needs exercise data for an applicant with a cardiac or pulmonary condition, it asks for a six-minute walk test recorded on this one-page result sheet. The test must follow American Thoracic Society guidelines, and in most cases the FAA looks for at least 400 meters walked for medical certification or clearance.

Who completes it

The provider who treats the applicant's cardiac or pulmonary condition runs the test, completes every item and signs with the evaluation date. The applicant's name, date of birth, applicant ID and PI number go at the top.

Submitting and payment

How to submit it
The applicant gives the sheet and any other supporting documents to the AME for upload into the FAA file, or sends it to AMCD or the Regional Flight Surgeon if an FAA letter directs.

Good to know

  • A grid records heart rate, SpO2, and dyspnea and fatigue on a 0 to 5 scale at baseline and at the end of each of the six minutes.
  • The sheet asks whether the applicant stopped or paused early and why, the time and distance walked if so, and whether supplemental oxygen or a rescue inhaler was used.
  • It also asks about walking difficulty or mobility aids, all current cardiopulmonary medications, symptoms at the end such as angina, leg pain or dizziness, and the provider's interpretation.
  • The sheet is listed among the AME Guide's disease protocols and dated 03/25/2026; the FAA's COPD Status Summary is a related document for pulmonary cases.
