FAA HIMS AME Checklist: Drug and Alcohol Recert

Pilots on a drug and alcohol monitoring special issuance see their HIMS AME for recertification, and the AME completes this one-page checklist from the in-office exam and the monitoring reports. It covers abstinence, test results, interval evaluations, the psychiatrist's report, aftercare counselor and chief and peer pilot reports, and any reports the Authorization letter adds, ending with the AME's recommendation.

Who completes it

The HIMS-trained AME completes, signs and dates it. Psychiatrists, aftercare counselors, chief pilots and peer pilots supply the reports it reviews.

Submitting and payment

How to submit it
The AME mails the signed checklist and the supporting documents, including the HIMS AME report, to the FAA's Aerospace Medical Certification Division within 14 days. Items 3 to 5 are reviewed by the AME and sent only if there are concerns.

Good to know

  • The HIMS AME face-to-face evaluation is required every 6 months for all classes, and the treating or HIMS psychiatrist report every 12 months unless the Authorization letter says otherwise.
  • Aftercare counselor reports are due every 3 months for first and second class; chief pilot and peer pilot reports are monthly for commercial first- or second-class pilots.
  • If every answer is in the clear column, the AME may issue with the Authorization letter's time limit. Any shaded answer means defer or contact the FAA and explain.
  • A relapse or withdrawal of authorization resets the time-in-phase date. The AME also records any step-down phase transition, from 2 to 3 or 3 to 4.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often does a HIMS pilot need a psychiatrist report?

Every 12 months for all classes, from the treating psychiatrist or a HIMS psychiatrist, unless the Authorization letter sets a different interval. The checklist asks whether the report is favorable with no anticipated treatment changes and no additional treatment or monitoring recommended.

Can the HIMS AME issue at recertification?

Yes, if every item on the checklist falls in the clear column, the AME may issue with the time limitation in the Authorization letter. If any single item falls in the shaded column, the AME must defer or contact the FAA and explain in the HIMS evaluation report.
