FAA Low Testosterone Status Summary for Pilots

Pilots treated for low testosterone or hypogonadism can bring this one-page FAA summary to the urologist, endocrinologist or physician who manages the condition. Five yes or no questions cover stability and side effects, phlebotomy, venous thromboembolism history, hematocrit above 54% on a CBC within 90 days, and clinical concerns, plus current medications. If every answer is NO, the pilot may qualify under the Low T CACI.

Who completes it

The urologist, endocrinologist or treating physician completes, signs and dates it, or writes a current, detailed Clinical Progress Note covering every item. The pilot fills in name, birthdate, applicant ID and PI number.

Submitting and payment

How to submit it
Pilots take the completed page and a copy of the current CBC to their AME for review and electronic upload. ATCS submit it to their Regional Flight Surgeon's office.

Good to know

  • Medications listed: anastrozole, clomiphene citrate (the AME confirms no visual side effects), HCG, oral or injectable testosterone, Testopel, tamoxifen, or other.
  • Any YES* answer or adverse change requires a current, detailed Clinical Progress Note addressing all items, and the AME must defer.
  • On deferral, the AME notes what caused it, explains the YES answers, and attaches the latest progress note and copies of any imaging reports.
  • The summary is not required, since clinical records may be submitted, but the FAA says it streamlines and significantly decreases review time.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What hematocrit triggers deferral on the FAA Low T status summary?

A hematocrit greater than 54% on a CBC from the previous 90 days falls in the YES* column, so the AME must defer. The pilot brings a copy of the current CBC with the summary so the AME can check it.

Is the Low T status summary required for the FAA?

No. Clinical records may be submitted instead, but the FAA says the summary streamlines and significantly decreases review time. If every answer is NO, the pilot may qualify under the CACI for low testosterone and the AME can issue at the exam.
