FAA CACI Colon/Colorectal Cancer Worksheet

Pilots treated for colon or colorectal cancer can be certified at the AME's office under the FAA's CACI program when the cancer was stage 0 to III at diagnosis, no high-risk features are present and the pilot is back to full activity with no treatment needed. This worksheet, updated 04/27/2022, also sets CEA and hemoglobin checks within the last 90 days.

Who completes it

The AME completes the worksheet from a current, detailed Clinical Progress Note written after a clinic visit with the treating physician or specialist, such as an oncologist or surgeon, no more than 90 days before the AME exam.

Submitting and payment

How to submit it
If all criteria are met, the AME issues, notes 'CACI qualified colon cancer/colorectal cancer' in Block 60 and keeps the documents on file without sending them to the FAA; otherwise the AME defers and submits them.
Who pays
The FAA does not set AME examination fees; the pilot pays the examiner, and the FAA recommends the usual and customary local fee for similar services.

Good to know

  • High-risk features that exclude the CACI include any chemotherapy or radiation, Lynch syndrome, FAP, positive margins, a rising or non-falling CEA, recurrence, non-adenoma pathology and distant metastasis.
  • CEA must have been under 5 ng/ml at diagnosis, and CEA within 90 days must be normal with no increase from previous levels.
  • A CBC within 90 days must show hemoglobin above 11 with no other significant abnormalities.
  • The note must exclude recurrence on colonoscopy or imaging and any symptoms suggesting metastasis, such as headache, vision change, focal neurologic signs, gait disturbance or cognitive change.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Can a pilot who had chemotherapy for colon cancer use the CACI?

No. Chemotherapy at any time, including neoadjuvant treatment, is a high-risk feature on the worksheet, as is radiation therapy. Either one means the AME must defer and send the documents to the FAA, which reviews the case rather than the AME issuing at the exam.

What labs does the FAA colon cancer CACI require?

A CEA under 5 ng/ml at diagnosis, a CEA within the last 90 days that is normal and has not risen from previous levels, and a CBC within 90 days with hemoglobin above 11 and no other significant abnormalities. The treating physician's note should include these results.
