Oklahoma medical forms for physicians, PAs and APRNs

Workers' comp release reports, Service Oklahoma placard and license forms, SoonerCare requests, sports physicals and medical marijuana recommendations, with who completes each.

Oklahoma's forms come from the Workers' Compensation Commission, Service Oklahoma, the Oklahoma Health Care Authority (SoonerCare), Oklahoma Human Services and the Medical Marijuana Authority (OMMA). Clinicians meet these most: the OSSAA Physical Evaluation Form, the OMMA Physician Recommendation Form for adult patients, the Physician's Report on Release and Restrictions (CC-Form-5), the Physical Disability Parking Placard Application (DPS 302DC) and the SoonerCare Prior Authorization Request (HCA-12A). This page lists all 24 Oklahoma forms, grouped by program.

The OSSAA physical needs a handwritten, dated signature; stamps are not accepted. For a placard, the patient can start online and Service Oklahoma emails the physician a link to complete their section, and applications expire 60 days after the physician signs. The patient uploads an OMMA recommendation within 30 days of the physician's signature. Only a physician may sign the school Certificate of Medical Exemption. Spanish versions exist for 3 of these forms.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign an Oklahoma OMMA physician recommendation?

An Oklahoma-licensed MD, DO or podiatric physician in good standing who is registered with OMMA. From January 1, 2026, physicians must also complete an initial medical marijuana education course, then annual CME. Oklahoma has no qualifying-condition list, so the condition section is optional. The form recommends a 2-year or 60-day temporary license and can certify the need for a caregiver. The patient uploads it in the OMMA MedPortal within 30 days. Minors need two physicians.

Who can certify an Oklahoma disability parking placard?

A physician licensed in medicine and surgery, osteopathic medicine, chiropractic, podiatric medicine or optometry; a licensed physician assistant; or a licensed and certified APRN. They complete Section 2 of the DPS 302DC for a mobility, respiratory, cardiac or vision condition. Placards are temporary (up to 6 months) or 5-year; lifetime permanent placards are no longer issued. The application expires 60 days after the physician signs.

What does the Oklahoma CC-Form-5 report?

The treating physician reports whether the claimant is released to regular or modified work or remains temporarily totally disabled, with specific restrictions and any need for continuing medical maintenance or vocational rehabilitation. The physician completes Sections I to III and certifies the report under penalty of perjury. Copies go to the employee or claimant and all other parties of record.

When can a student get an OSSAA sports physical?

No earlier than May 1 of the preceding year. A qualified physician, physician assistant or advanced practice nurse covered by professional liability insurance completes the physical examination and clearance. The signature must be handwritten and dated; stamps are not accepted. The student files the form, with parental consent, at the school before taking part in OSSAA activities.
