Oklahoma 04AF008E Resource Parent Medical Exam

Every Oklahoma resource parent applicant (foster or adoptive) needs a physical exam before approval, and the examining physician reports it on this two-page form. The exam explores whether the applicant has conditions that prevent or limit them from safely providing daily care for children who may have medical or behavioral needs.

Who completes it

The examining physician completes the whole report: when the patient was first and last seen, vitals, medical history, current medications, the care questions and comments, then signs with printed name, address and phone number.

Submitting and payment

How to submit it
Return the completed report to the Oklahoma Human Services resource family worker or agency handling the application.

Frequently asked questions

Is a physical exam required for Oklahoma resource parents?

Yes. The form states that each applicant is required to have a physical exam before receiving approval as a resource parent. The examining physician documents it on 04AF008E, the Medical Examination Report.

What does the physician answer on 04AF008E?

Whether the patient has any condition that impairs their ability to safely provide daily care for children through the next year and possibly into the children's adulthood, and whether emotional or behavioral issues have been addressed, each with an explanation if yes, plus comments on emotional or physical health.

Does 04AF008E ask when I last saw the patient?

Yes. The patient information section asks when the patient was first seen and when they were last seen, excluding the current visit, before the exam data: height, weight, blood pressure and pulse. Medical history and a medication list with dosage and reason follow.
