Oklahoma DDS-5 Referral for Treatment or Examination

Oklahoma Developmental Disabilities Services sends this five-page referral with a service recipient to each health care appointment. Provider staff describe current medications, allergies, symptoms and recent illness or admissions; the clinician then records diagnosis or test results, recommendations, treatments, medication changes, monitoring instructions for staff and tardive dyskinesia monitoring.

Who completes it

The health care provider seen at the appointment completes the Healthcare Provider/Medical Facility Section (pages 3 to 5) and signs. DDS provider staff complete the client and health information sections beforehand and sign as the person completing the form.

Submitting and payment

How to submit it
Within one business day, copies go to the health care provider, the home record and the DDS case manager, as listed in the routing box.

Good to know

  • You may attach a clinical note or results instead of writing out the diagnosis and diagnostic test findings.
  • List every medication being added, changed or discontinued with the reason; staff attach the current MAR or medication list in the health information section before the visit.
  • The tardive dyskinesia block asks whether the person takes a medication with TD risk, including metoclopramide, with the last assessment date and score; give a copy of the current assessment.
  • Psychiatric visits use a separate form, 06HM073E (DDS-73). The current DDS-5 edition is dated 03/26/2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
