Oklahoma 08AD065E Energy Assistance Medical Release

Oklahoma Human Services' Energy Assistance program uses this two-page form to decide eligibility for life-threatening energy crisis assistance. A licensed health care provider answers yes or no questions on whether the patient needs prescribed equipment that runs on electricity, or refrigerated medication, and whether losing power would be life-threatening.

Who completes it

A licensed health care provider completes the medical equipment, refrigerated medication and additional information sections and signs the attestation with institution name. The client, guardian or parent signs the consent at the top, and the OKDHS worker adds contact information.

Submitting and payment

How to submit it
Return the completed form to the OKDHS worker named in the Worker Contact Information box, by the email or fax listed there.

Good to know

  • Equipment questions cover whether it needs electricity, whether an outage would be life-threatening, whether it has battery backup, whether the battery would last, and whether it can be charged elsewhere.
  • Battery questions accept 'I don't know' as an answer, so you do not need to guess about device specifications.
  • The client's consent expires six months from signing, and clients may supply the information themselves instead of using the form.
  • A Spanish version, 08AD065S, carries the same date as the English edition, 3/16/2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does the provider attest to on 08AD065E?

Does 08AD065E ask for a diagnosis?

No. It asks yes or no questions about a condition requiring prescribed equipment or medication, electricity and battery backup, refrigeration and life-threatening risk. An additional information box lets the provider add anything else relevant to the energy crisis request.

Is the Oklahoma energy assistance medical form available in Spanish?

Yes. Oklahoma Human Services posts 08AD065S, the Spanish version of the Request for Release of Medical Information, with the same March 2026 date as the English 08AD065E. Both versions include the client consent, the equipment and refrigerated medication questions, and the provider attestation.
