Oklahoma Autism Designation Enrollment Form

Oklahomans with an autism spectrum disorder can voluntarily add an autism designation to their driver license or state ID record so law enforcement officers and emergency medical professionals can identify and communicate with them. Service Oklahoma requires a provider's signed certification of the diagnosis in Section 2. The individual chooses whether the designation appears on the credential, is shared with law enforcement, or both.

Who completes it

A licensed primary care physician, licensed psychologist or licensed psychiatrist completes Section 2, certifying the diagnosis with license type and number. The individual completes Sections 1, 3, 4 and 5, including the display choice and optional emergency contact.

Submitting and payment

How to submit it
The individual, not the provider, submits the completed form to Service Oklahoma.

Good to know

  • The designation is requested under 47 O.S. §6-125 and is voluntary; it is not required to obtain or renew a driver license or state ID card.
  • Section 4 lets the individual list communication considerations visible to law enforcement: prefers written communication, may need more time to respond, may avoid eye contact, sensory sensitivity, or other.
  • The individual can update or remove the designation at any time through procedures set by Service Oklahoma.
  • Service Oklahoma has a matching Epilepsy Designation Enrollment form with the same provider certification section.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify autism for the Oklahoma designation?

Section 2 must be completed by a licensed primary care physician, licensed psychologist or licensed psychiatrist. The provider certifies that the person named in Section 1 has been diagnosed with an autism spectrum disorder and adds printed name, license type, license number, signature and date.

Does the autism designation have to appear on the Oklahoma license?

No. In Section 3 the individual chooses one option: displayed on the credential, communication considerations shared with law enforcement, or both. The designation is voluntary and can be updated or removed later through Service Oklahoma.

Does the provider fill in the communication considerations?

No. Section 4, the emergency contact and communication considerations, is completed voluntarily by the individual, and anything entered there is accessible to law enforcement officers. The provider's part is limited to the Section 2 certification of diagnosis.
