Oklahoma ODH 216-A: Certificate of Medical Exemption

Oklahoma children with a medical contraindication to required vaccines can attend school, child care or Head Start without them when a licensed physician certifies the condition on this certificate. The physician states the condition that would endanger the child's life or health, and the vaccines covered are checked. The 2026 layout separates the medical certificate from the parent's religious and personal exemption certificate.

Who completes it

Only a physician may sign Section 1, and the form states that physician assistant, nurse or ARNP signatures will be denied. The parent completes the child, school and demographic fields and signs the acknowledgment.

Submitting and payment

How to submit it
The completed certificate is mailed to the OSDH Immunization Service, either directly or through the school, and the school, child care facility or Head Start keeps a copy on file.

Good to know

  • The exemption expires after 6th grade; a new exemption request is required before the child enrolls in 7th grade.
  • Lost immunization records are not grounds for an exemption; the instructions send parents to the local health department or family physician to rebuild a record.
  • Every section must be completed and entries must be legible, or the form will be returned.
  • Vaccines that can be checked: DTaP/Td/Tdap, hepatitis A, hepatitis B, Hib, MMR, pneumococcal, polio and varicella, or All.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a nurse practitioner sign an Oklahoma medical exemption?

How long does an Oklahoma medical exemption last?

The parent acknowledgment on the April 2026 form states that the exemption expires after 6th grade and that a new exemption request is required before the child enrolls in 7th grade. Copies must stay on file at the school, child care facility or Head Start and be available for review.
