Oklahoma HCA-27 Physician Certification for Transport

SoonerCare requires this certification when a member needs non-emergency transport by ground ambulance, fixed-wing air ambulance or rotary air, moving between an originating and an accepting facility. The ordering provider documents diagnosis, ability to sit up, airway and oxygen status, the reason for transfer and transport team needs. Signing affirms that any other method of transportation is contraindicated.

Who completes it

The ordering provider completes and signs it; the form states it must be signed by a physician, NP or PA. Contacts for the originating facility, accepting facility and transportation provider are recorded on it.

Submitting and payment

How to submit it
Fax the completed form to OHCA; the fillable PDF also carries an Adobe Sign signature field for electronic signing.

Good to know

  • If the transport takes the member out of Oklahoma for care, the form directs the provider to also complete an HCA-65 prior authorization request for out-of-state care.
  • Ambulatory questions ask whether the member can sit up in a chair, for how long, and what illness and movement limitations keep them from getting out of bed.
  • Airway questions cover patent airway, oxygen use (continuous or intermittent) and ventilator dependence; the form also asks about special transport teams such as NICU or ALS.
  • Nursing home ambulance transport uses a different OHCA form, the HCA-40 Nursing Home Ambulance Transportation Form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Oklahoma HCA-27?

The form states it must be signed by a physician, nurse practitioner or physician assistant. The ordering provider signs and dates it, affirming that the information is correct and that using any other method of transportation is contraindicated for the member.

What if the SoonerCare member is being transported out of state?

HCA-27 asks whether the transport is related to care outside Oklahoma. If it is, the form directs the provider to form HCA-65, the Out of State Prior Authorization Request, which must be completed to request prior authorization for the out-of-state care.

What transport types does HCA-27 cover?

Three: ground ambulance, fixed-wing air ambulance and rotary air transportation. The provider also indicates whether a guardian or medical escort will travel, whether a special team such as NICU or ALS is needed, and whether a transportation provider has agreed to take the member.
