Oklahoma 302DC: Physical Disability Parking Placard

Oklahoma residents use form 302DC to apply to Service Oklahoma for a temporary or five-year physical disability parking placard. In Section 2 the physician certifies a qualifying mobility, respiratory, cardiac, vision or limb condition and answers whether it affects safe driving. Service Oklahoma considers new or renewal applications only within 60 days of the physician's signature.

Who completes it

The applicant completes Section 1 and signs. A physician licensed in medicine, osteopathy, chiropractic, podiatry or optometry, a licensed physician assistant, or a licensed and certified APRN completes Section 2 in full.

Submitting and payment

How to submit it
The paper form is mailed or dropped off at Service Oklahoma. Alternatively, the patient applies online and the physician receives an email link to complete their portion, or the two apply together in the office.
Who pays
Service Oklahoma issues placards at no charge; the form does not address payment for the physician's section.

Good to know

  • The driving question asks whether the condition would affect safe operation of a motor vehicle; signing the form allows Service Oklahoma to review the applicant's driving ability under 47 O.S. 6-119.
  • Temporary placards last up to six months with a physician-set expiration date. Lifetime permanent placards are no longer issued; the long-term option is a five-year placard.
  • Section 2 lists eight qualifying conditions, from walking under 200 feet without resting and portable oxygen to legal blindness and missing limbs that impair mobility.
  • Service Oklahoma processes applications in about 20 business days. Disability plates require first holding a five-year placard.
