Arkansas 10-336 Disability Placard Certification

Arkansas issues special license plates and parking certificates (placards) to people with disabilities after a licensed provider certifies the condition on form 10-336. In Part 1 the provider marks the disability as permanent or temporary and checks one or more conditions from A through L. Page 2 explains the parking rules, penalties and expiration periods that apply to the holder.

Who completes it

A licensed physician, physician assistant, physical therapist or advanced practice registered nurse completes Part 1, including their license number, the patient's details and the qualifying conditions, and signs. Part 2 is only for organizations that transport people with disabilities.

Submitting and payment

How to submit it

Good to know

  • Conditions include being unable to walk 100 feet without rest, assistive device use, severe lung disease, portable oxygen, AHA Class III or IV cardiac limits, spinal cord injury, amputation, spina bifida and multiple sclerosis.
  • If none of conditions A through L applies, the provider lists the permanent medical condition that substantially affects the person's mobility.
  • A permanent certificate expires four years from the end of the month issued and renews without recertification. A temporary certificate expires three months from the end of the month issued.
  • Temporary placards need the person's Social Security, driver's license or state ID number. Giving false information to obtain a plate or certificate is a Class A misdemeanor.
