Idaho medical forms clinicians complete, by program

The 11 Idaho forms that ask for a clinician's input, from the IHSAA sports physical and ITD 3392 placard to Medicaid prior authorization, POST and hunting permits.

This page lists the 11 Idaho forms that ask for a clinician's input, grouped by program. The ones you meet most are the IHSAA Pre-Participation Physical Examination and Consent for high school athletes and the ITD 3392, which certifies a patient for disability plates or a special parking placard under Idaho Code 49-117. Idaho Medicaid has prior authorization forms for home health and for surgeries and procedures, and DHW publishes the school immunization exemption form.

On the ITD 3392, a physician, PA or advanced practice professional nurse certifies, and a temporary placard runs 1 to 6 months with the expiration date you set. A medical exemption from school immunizations must be completed by a licensed physician. Home health PA requests need a signed, dated physician order, with no verbal orders or signature stamps. The POST isn't posted online; providers get it through the Idaho Healthcare Directive Registry. 3 forms here have a Spanish version, including the IHSAA physical.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify an Idaho disability placard?

A licensed physician, physician assistant or advanced practice professional nurse completes the Medical Certification section on page 1 of the ITD 3392, confirming the patient meets Idaho Code 49-117. For a temporary placard, which lasts 1 to 6 months, the certifier gives a specific expiration date. The same form covers permanent placards and disability license plates. The applicant mails it to the ITD Special Plates Unit or takes it to a county office.

Who can sign a medical exemption from Idaho school immunizations?

A licensed physician. The Immunization Requirements Exemption Form for preschool and K-12 has a medical exemption section that the physician signs. Parents can claim religious or other exemptions themselves, on the form or with any signed written statement. The completed form goes to the school or preschool. DHW publishes it in English and Spanish.

Where do I get the Idaho POST form?

From the Idaho Healthcare Directive Registry. DHW doesn't post a public PDF of the Physician Orders for Scope of Treatment; providers get it through the registry, and anyone can request a copy from the registry. A physician, APRN or PA completes the orders with the patient, covering resuscitation and scope of treatment, and the form may be stored in the registry. Idaho Code 39-4512A governs it.

Who can certify an Idaho disabled hunting or fishing license?

A physician, physician assistant or nurse practitioner. The Certification of Permanent Disability supports a resident disabled combination or fishing license at a reduced fee under Idaho Code 36-406(g). The BA-116 supports a permit to hunt from a motor vehicle for a hunter who has lost use of the lower extremities or hands, or can't walk 200 feet unassisted; practitioners licensed in the US or Canada may sign it. Both go to any IDFG office.
