Idaho CNP Medical Statement for Meal Accommodation

Idaho schools, child care centers and other Child Nutrition Program sites use this statement when a student or participant needs a special meal or accommodation because of a medical condition or disability. The medical authority describes the impairment and the major life activity it limits, the diet prescription, texture, foods to omit with alternatives, and any adaptive equipment.

Who completes it

A licensed physician, physician assistant, nurse practitioner, naturopathic medical doctor or dentist signs line 14, and for CACFP, NSLP and SBP a registered dietitian may sign. The parent or guardian fills in lines 1 to 6 and signs lines 18 to 21.

Submitting and payment

How to submit it
Give the completed statement to the parent to return to the school or program site that will serve the meals.

Good to know

  • Line 8 asks how the condition affects eating. The instructions give the example of a peanut allergy causing anaphylaxis that requires epinephrine and an emergency room visit.
  • Texture choices are regular, chopped, ground, pureed, liquid or other. 'Other' can specify prescribed liquid consistencies such as thin, nectar, honey or pudding.
  • The form notes that since the ADA Amendments Act of 2008 most physical and mental impairments count as disabilities, so sponsors focus on equal participation rather than eligibility.
  • The statement should be updated periodically to reflect changes in the participant's medical or nutritional needs. A Spanish version is posted on the Idaho CNP page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can a dentist sign Idaho's CNP Medical Statement?

Yes. The form lists a licensed physician, physician assistant, nurse practitioner, naturopathic medical doctor or dentist as the medical authorities who may sign line 14. For the CACFP, the National School Lunch Program and the School Breakfast Program, a registered dietitian may also sign.

What goes on the Idaho statement's diet prescription line?

Line 9 describes the specific diet or accommodation in enough detail for staff to carry it out, for example that all foods must be liquid or pureed and the child cannot eat solids. Lines 11 and 12 then list foods to omit, such as fluid milk, and alternatives, such as lactose-reduced or soy milk.
