Idaho ITD 3392: Disability Plates and Placard Form

Idaho residents use ITD 3392 to apply for disability license plates, a permanent placard or a temporary placard. The Medical Certification section on page 1 confirms that the patient meets one or more of the disability requirements in Idaho Code 49-117. A clinician meets the form when a patient needs accessible parking, and the Idaho Transportation Department or county DMV issues plates or placards from it.

Who completes it

The applicant completes the applicant information, placard count and, for plates, the vehicle section. A licensed physician, physician assistant or advanced practice professional nurse completes and signs the required Medical Certification.

Submitting and payment

How to submit it
Applications that include a plate go by mail or email to the ITD Vehicle Services Special Plates unit. Placard-only applications can be mailed there or taken to the local county DMV.
Who pays
The form does not address payment for the medical certification; plate fees, including optional personalized plate fees, are billed to the applicant.

Good to know

  • For a temporary disability, the certifier writes a specific expiration date. Temporary placards are issued for one to six months.
  • The practitioner's National Provider Identifier is a required field, along with printed name, address, office phone and signature.
  • Only permanent disability applicants choose one or two placards. The current version is Rev. 8/26, so older printed copies should be replaced.
  • Idaho Code 49-456 makes false statements or concealment of a material fact on the application unlawful, and misuse of a plate or placard is an infraction.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Idaho ITD 3392 medical certification?

A licensed physician, physician assistant or advanced practice professional nurse. The certifier attests that the patient meets one or more disability requirements in Idaho Code 49-117, marks temporary or permanent, and adds an NPI, address, office phone, signature and date.

How long is an Idaho temporary disability placard valid?

Temporary placards are issued for one to six months. Instead of choosing a length, the certifier writes the specific date the temporary disability placard should expire. A patient still impaired at that point needs a new application with a new medical certification.

What privileges come with an Idaho disability placard?

When the person with the disability is present, the vehicle may park at meters without paying, park without time limits in time-restricted zones, and buy fuel at a full-service pump at the self-service price. Zones where all parking is banned still apply.
