Idaho POST: Physician Orders for Scope of Treatment

Idahoans with a serious illness or approaching the end of life can have specific medical orders on resuscitation and scope of treatment written on this form, which complements the advance directive. The Department of Health and Welfare does not post a downloadable copy: patients and providers request the form, and providers can create and view POSTs through the Idaho Healthcare Directive Registry.

Who completes it

A physician, APRN or PA completes the orders with the patient, and both the patient and the provider sign the form.

Submitting and payment

How to submit it
Request a copy from the Idaho Healthcare Directive Registry by email, or create it through the registry's online platform; completed POSTs may be stored in the registry, where providers can view them.

Good to know

  • The registry's resource page offers tip sheets on creating accounts, uploading documents and creating POST forms online.
  • Idaho's healthcare directive registry moved from the Secretary of State to the Department of Health and Welfare.
  • The POST is authorized by Idaho Code 39-4512A and complements the Idaho Advance Directive, which combines a durable power of attorney for healthcare and living will.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
