Maine DAVE: Death Certificate Medical Certification

Maine certifies deaths electronically in DAVE, the Database Application for Vital Events, through its Electronic Death Registration System module. The funeral director or vital records staff enter the decedent's personal information and the medical certifier completes the medical certification. Once both parts pass all validation rules, the record is registered and assigned a State File Number, and certified copies can be issued.

Who completes it

A medical certifier (MD, DO, PA or certified nurse practitioner) or a medical examiner completes and certifies the medical section: pronouncement, place of death, cause of death, other factors and the certifier page. Facility staff may enter this information for the provider to review and certify; the decedent page is read-only for providers.

Submitting and payment

How to submit it
Log in to DAVE, find or start the case, complete each Medical Certification screen, validate the pages and click Certify on the affirmation. Certifiers first register using the DAVE Enrollment Form for Medical Certifiers & Staff.

Good to know

  • The cause-of-death screen rejects abbreviations, and DAVE will not approve terms on the state's referral list, such as fall, overdose, sepsis or injury; those cases go to the Medical Examiner.
  • The manner must be natural unless the Office of Chief Medical Examiner has been contacted and issued a release number. Any trauma case is referred through the Refer to Medical Examiner link.
  • To change a certified case, use Un-certify, edit and certify again. The link disappears once the record is registered, after which medical items are changed through an electronic amendment.
  • If the time of death is unknown, enter 9999 and select unknown. The certifier page asks whether the body was viewed after death, which is required for nursing home patients.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify a death in Maine's DAVE system?

Enrolled medical certifiers with the title MD, DO, PA or certified nurse practitioner, and medical examiners. Facility staff can hold a non-certification role and enter the pronouncement, place and cause of death, but the provider must log in, validate the certifier page and click Certify to complete the medical certification.

How does a Maine clinician get a DAVE account?

Complete the DAVE Enrollment Form for Medical Certifiers & Staff, choosing the medical certifier or non-certification staff role, entering a professional license number and the training received, and signing the confidentiality statement. The DAVE help desk handles usernames, passwords, error overrides and account registration.

When is a Maine death record registered?

Only after both the personal information and the medical certification are entered in DAVE and pass all validation rules. The record then receives a State File Number, and copies of the death certificate can be issued by vital records, the municipality where the death occurred or the decedent's municipality of residence.
