Maine Consulting Physician End-of-Life Care Form

Maine's Death with Dignity Act requires a second physician's opinion before a prescription. The attending physician refers the patient to a consulting physician, who uses this one-page form to record the date and time of their examination, confirm they reviewed the relevant medical records, name the terminal diagnosis, and confirm that the patient is competent, acting voluntarily and making an informed decision.

Who completes it

The consulting physician completes Sections A to D, including their name and license number in Section D, and signs. Section B identifies the referring or attending physician, who files the form.

Submitting and payment

How to submit it
The consulting physician gives the completed form to the attending physician, who keeps the original in the patient's medical record and sends a copy to the State Registrar, Office of Data, Research, and Vital Statistics.

Good to know

  • Maine CDC links it as the Consulting Physician Form, but the printed title is Consulting Physician End-of-Life Care. The fillable version is revised 06/2024.
  • Section C has four boxes: terminal disease diagnosis, competence, informed decision, and a voluntary request for medication to end life in a humane and dignified manner.
  • The attending physician's own report lists the consulting physician by name and records the referral, so the two forms are filed together with the State Registrar.
  • Participation is voluntary for every physician and health care professional, according to Maine DHHS's Death with Dignity questions and answers.
