Indiana DRIVE Death Medical Certification

Indiana does not use a paper medical certification for death records: physicians, NPs and PAs certify cause of death online in DRIVE, the Database Registration of Indiana's Vital Events. The medical certifier completes the medical section of the death record, and coroners use the same system for their cases. The Indiana Department of Health publishes user guides, recorded training and an amendment guide.

Who completes it

The medical certifier, a physician, NP or PA, or the coroner completes the medical certifier section of the death record in DRIVE.

Submitting and payment

How to submit it
Complete it online in DRIVE: create an Access Indiana account, then submit an e-request through the DRIVE tile to link a DRIVE profile.

Good to know

  • Enrollment is a two-step process: an Access Indiana account first, then a DRIVE e-request submitted from the Access Indiana tile.
  • Use Start/Edit Case to open a record for the first time; a separate guide explains when to use Locate Case instead.
  • DRIVE also has fetal death and terminated pregnancy modules, with separate recorded training for medical certifiers and coroners.
  • Corrections after filing follow the Amendment Guide for Coroners and Medical Certifiers; technical problems go through a VR Helpdesk ticket.
