New York State DAVE: Death Certificate Certification

Deaths and fetal deaths that occur in New York State outside New York City are registered electronically in DAVE, the Database Application for Vital Events, formerly called EDRS. Medical certifiers complete the cause-of-death certification online through the Health Commerce System portal; there is no paper form to download. Deaths in New York City go through NYC eVital instead.

Who completes it

Physicians (MD or DO), nurse practitioners and physician assistants licensed in New York complete the medical certification under the EDRS Medical Certifier role; medical examiners and coroners have their own roles. Funeral directors and local registrars complete their parts of the record in the same system.

Submitting and payment

How to submit it
The certifier signs in to the Health Commerce System, opens DAVE from My Applications, selects the office responsible for the case and completes the certification online.

Good to know

  • Access needs an HCS account plus an EDRS role assigned by the HCS Coordinator at the certifier's facility; each person holds only one EDRS role per facility or office.
  • A private-practice physician who does not share cases with staff, or who works as a coroner's physician, can create an Independent Medical Certifier profile without an HCS Coordinator.
  • On first login a medical certifier selects a title, then the office handling the case; a re-sync link helps when an office affiliation is missing.
  • Coordinators must remove a user's EDRS role when the person leaves or changes jobs. Quick reference guides sit under the Help menu inside DAVE.

Frequently asked questions

How does a New York physician get access to DAVE?

Request a Health Commerce System account through the Sign Up Here link on the HCS login page, then have the HCS Coordinator at your facility assign the EDRS Medical Certifier role. Physicians in private practice who work alone can instead create an Independent Medical Certifier profile when they first open DAVE.

Do New York City deaths go through DAVE?

No. DAVE covers deaths and fetal deaths that occur in New York State outside New York City. Deaths in New York City are certified through NYC eVital, the city's own electronic vital records system, so certifiers working in both areas use both systems.
