NYC eVital Death Certificate Medical Certification

New York City reports deaths and issues death certificates through eVital, the Health Department's system for life events, which replaced EVERS. Pre-authorized medical facilities report deaths, and enrolled certifiers certify the cause of death online. Physicians meet it whenever they certify a death occurring in New York City; deaths elsewhere in New York State go through the state's separate system.

Who completes it

A physician or other authorized medical certifier at an enrolled facility completes the medical certification. Funeral directors handle their own parts of the case in eVital, such as cremation clearance and personal amendments.

Submitting and payment

How to submit it
Certify online in eVital or with the NYC eVital Certify app on a personal Android or iPhone device; certifiers authenticate each certification with facial recognition.

Good to know

  • Only authorized users can enter data; hospital staff who need access contact their facility's eVital Facility Administrator, who enrolls certifiers on site.
  • Facility Administrators themselves enroll in person at the Health Department, and former EVERS users linked to several facilities enroll with an EVERS Migration Key.
  • Sign-in uses NYC.ID single sign-on, which replaced NYCMED, and scheduled maintenance runs on Thursday evenings.
  • Written guides cover reporting deaths, death amendments, amendments for death queries and the January 2020 death certificate changes.

Frequently asked questions

How do physicians get access to NYC eVital?

Through their facility. Each enrolled medical facility has an eVital Facility Administrator who enrolls users, including deputy administrators and certifiers, on site. Hospital staff who need access should contact that administrator; facilities that are not yet authorized must request permission from the Health Department's eVital team.

Can I certify a death on my phone in NYC?

Yes. Certifiers can use the NYC eVital Certify app on a personal Android or iPhone device. Certification uses facial recognition instead of fingerprints; the app warns when connectivity is too weak and allows two more attempts if the first photo submission fails.
