Idaho Immunization Exemption Form, Preschool and K-12

Idaho law lets a parent or guardian claim an exemption from preschool and K-12 immunization requirements for medical, religious or other reasons, and DHW's Immunization Program provides this form to document it. A medical exemption must be completed by a licensed physician, while religious or other exemptions are claimed by the parent.

Who completes it

A licensed physician completes and signs the medical exemption section. For religious or other reasons, the parent or guardian completes the form or provides any signed written statement instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
