Idaho Medicaid Surgery & Procedure Prior Authorization

Idaho Medicaid uses this one-page form to request prior authorization for surgeries and procedures that are not reviewed by its utilization management contractor, Telligen. A surgeon's or physician's office meets it when a planned procedure needs Medicaid approval outside Telligen's scope. The office lists CPT codes, quantities and dates, identifies the physician and any facility by NPI, and faxes the form with supporting medical records.

Who completes it

The physician's office completes the entire form: participant details, physician and submitter contact, facility information, and the requested CPT codes with modifiers, quantities and dates of service. The participant does not complete any part.

Submitting and payment

How to submit it
Fax the completed form with all supporting medical documentation to the number printed on it. Request status can be checked online at idmedicaid.com under Authorization Status using the provider's NPI.

Good to know

  • When the procedure takes place in a facility, separate authorization numbers are created for the facility and the surgeon from the NPIs entered, so give both. Do not submit Tax ID numbers.
  • For an in-office procedure, leave the facility name blank and mark only the 'N/A, In Physician's Office Procedure' box.
  • Fax documentation that supports medical necessity, such as physician notes, consultations, operative reports or medical records from the last six months.
  • Most outpatient, imaging and inpatient requests go through Telligen. Idaho also posts separate PA forms for laboratory, therapy, home health, early vision exams and dental anesthesia.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When do I use the Idaho Medicaid Surgery & Procedure form instead of Telligen?

Use it for surgeries and procedures that Telligen, Idaho Medicaid's utilization management contractor, does not review. Most outpatient, imaging and inpatient prior authorizations go through the Telligen portal; this Department form covers the remaining procedures and is faxed with the supporting medical records.

What records go with the Idaho surgery prior authorization form?

The form asks for all medical documentation that supports medical necessity, for example physician notes, consultations, operative reports or medical records from the last six months. Fax them with the completed form, which itself identifies only the participant, physician, facility, CPT codes, quantities and dates.
