West Virginia Medicaid General Drug PA Form

West Virginia's Bureau for Medical Services uses this one-page form for Medicaid pharmacy prior authorization when no drug-specific form applies. The Rational Drug Therapy Program at the WVU School of Pharmacy receives it. Because West Virginia carves pharmacy out of managed care, the form applies to all West Virginia Medicaid members, whatever their health plan.

Who completes it

The prescriber completes the patient, prescriber and drug details, diagnosis, previous treatment history and other pertinent information, then signs. The signature line reads prescriber or pharmacist, and the pharmacy section is filled in only if applicable.

Submitting and payment

How to submit it
Fax the completed form to the Rational Drug Therapy Program; its fax and phone numbers are printed at the top of the form.

Good to know

  • The form asks for the 11-digit WV Medicaid ID, the prescriber's 10-digit NPI, drug name, strength, route of administration, directions, duration and the ICD diagnosis code if available.
  • Pharmaceutical samples are not considered when evaluating the member's medical condition or prior prescription history for drugs that require prior authorization.
  • The signature attests that the request is medically necessary, does not exceed the member's needs, and is documented in the medical record, which must be available on request.
  • The form covers pharmacy only. Medical service prior authorizations go through Acentra Health's Atrezzo portal, and about 125 drug-specific forms and criteria are posted on the BMS criteria page.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does PA approval guarantee West Virginia Medicaid payment?

No. The form's important notes state that preauthorization for medical necessity does not guarantee payment. The prescriber's signature also certifies that the request is documented in the medical record, and medical or pharmacy records must be made available on request.

When should a prescriber use the West Virginia general drug PA form?

Use it when a drug needs prior authorization and no drug-specific form applies. The Bureau for Medical Services posts about 125 drug-specific forms and criteria documents on its prior authorization criteria page; check there first, then use the general form.

Can the West Virginia drug PA form be signed electronically?

Yes. The signature block has a checkbox for an electronic signature, and the attestation says the signature may be manual or electronic. Either way, the signer certifies medical necessity and that the request is documented in the medical records.
