Alabama Medicaid Form 342: Prior Review Request

Alabama Medicaid's Form 342 is the one-page request for prior review and authorization of non-pharmacy services, from surgery and DME to home health, therapy, transportation and private duty nursing. The requesting provider codes the service type, diagnoses and prognosis, lists each service line with dates, place of service, procedure code, modifier and units, adds a clinical statement and signs a certification of medical necessity.

Who completes it

The requesting provider, such as a physician, DME supplier, home health agency or therapist, completes the form and signs; a rendering provider block identifies who will deliver the service. The signer certifies that the form and any attached letterhead statement were completed by them or their employee and reviewed by them.

Submitting and payment

How to submit it
Send the form with supporting documentation to Gainwell at the address printed on it, or submit through the Medicaid Interactive Web Portal. Radiology and cardiology requests are accepted only through the web portal.

Good to know

  • For PT, OT, respiratory, speech, audiology, psychotherapy, oxygen certification, home health and transportation requests, attach a current plan of treatment and progress notes on necessity, effectiveness and goals.
  • The clinical statement must include prognosis and rehabilitation potential, and the provider certifies that a physician-signed order is on file when one applies.
  • Psychiatric and inpatient stay requests need no procedure code. DME requests mark the item new or used, and ambulance requests add transport and transport reason codes.
  • Some services need an attachment: Form 384 for wheelchair evaluations, 386 for modification or repair, 360 for oxygen and 480 for ACD evaluations. Changes use Form 471.
