Alabama Medicaid Form 369: Pharmacy PA Request

Alabama Medicaid prescribers use Form 369 to request prior authorization for non-preferred drugs and other drugs that require PA when no drug-specific form applies. Page 1 collects patient, prescriber and clinical details, previous drugs with start and end dates, and medical justification. Pages 2 and 3 add drug-specific questions. Acentra Health reviews requests for the Alabama Medicaid Agency.

Who completes it

The prescribing practitioner completes the patient, prescriber and clinical sections and signs a certification that the treatment is indicated, necessary and within Medicaid guidelines and that they will supervise it. The dispensing pharmacy section may be completed by the pharmacy.

Submitting and payment

How to submit it
Fax or mail the form to Acentra Health, or use the online PA request form linked from the Alabama Medicaid and Acentra Health websites; online requests follow paper request rules. Verbal requests are accepted for some drug classes when prior-use requirements are met.

Good to know

  • If the requested brand has an exact generic equivalent, FDA MedWatch Form 3500 must also be sent to Acentra Health along with the PA request.
  • Medications received through coupons or samples are not acceptable as justification, and samples or vouchers do not count toward stable therapy requirements.
  • Pages 2 and 3 cover classes such as sustained-release opioids, antipsychotics, CGRP migraine agents, specialized nutritionals, Xolair, hereditary angioedema agents and Xenical; page 1 may be sent alone otherwise.
  • Diagnoses can be written out or coded, but ICD-10 codes add specificity and usually expedite review. Use a separate form for each drug or nutritional requested.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When should Alabama Medicaid Form 369 be used?

Use it for a non-preferred drug or a drug that requires prior authorization when no drug-specific form applies. DMARDs and biologics, Synagis, hepatitis C antivirals, growth failure agents, smoking cessation agents and opioid dependence agents each have separate PA forms and instructions.

What happens when a pharmacy claim hits Alabama's electronic PA?

For drug classes in the electronic PA program, the system checks the patient's medical and pharmacy claims history. If criteria are met, the claim is approved automatically. If not, the pharmacy is told to submit a manual paper or online request; that rejection is not a PA denial.

What counts as medical justification on Form 369?

Documentation supporting the prescriber's choice of treatment, such as records from the patient chart or peer-reviewed literature, listed in the justification field with extra pages attached if needed. Previous drugs should show generic, brand or OTC status, reason for discontinuation and therapy start and end dates.
