Puerto Rico medical forms for physicians: CESCO, DTOP and SINOT

Driver's license, eye exam, parking placard and seat belt or tint certifications for CESCO, plus the SINOT disability certificate, with who signs each one.

Puerto Rico's clinician forms come from two agencies. The Departamento de Transportación y Obras Públicas (DTOP) issues the driver and parking certifications through CESCO, and the Department of Labor and Human Resources (DTRH) runs SINOT, the non-occupational disability insurance program. Physicians see the Medical Certification for a Driver's License Certificate (DTOP-DIS-260) most, since every new license or learner's permit needs one, followed by the removable parking placard application (DTOP-DIS-010). This hub lists 6 forms, including the eye doctor's DTOP-789 and the SINOT SI-1 Part C.

Plan on Spanish: 6 of these forms are in Spanish, and the DTOP forms exist only in Spanish, so the English names shown are translations. Most are flat PDFs that the patient presents at CESCO. Only a physician licensed in Puerto Rico may sign the DTOP-DIS-260, and CESCO accepts no blanks or alterations. The placard and the seat belt or window tint exemption (DTOP-375) need a specialist physician. On the SI-1, each treating physician, chiropractor or psychologist completes a separate Part C.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the DTOP-DIS-260 driver's license medical certification?

A physician licensed to practice in Puerto Rico who personally examines the applicant. The physician checks visual acuity and physical and mental condition and certifies whether the person is fit to drive under Ley 22-2000. Every new license or learner's permit needs one. The form is a single legal-size page, Spanish-only, and CESCO accepts no blanks or alterations. The applicant presents it at CESCO with the license application.

Which doctors can certify a Puerto Rico parking placard on the DTOP-DIS-010?

A specialist physician (médico especialista) completes the Certificación de Elegibilidad on page 2. For fibromyalgia, chronic fatigue syndrome, lupus or rheumatoid arthritis, the certifier must be a physiatrist or rheumatologist. The physician certifies one of the qualifying conditions listed in Ley 22-2000, such as paralysis, amputation, severe cardiac or respiratory disease or multiple sclerosis, for a permanent or temporary removable placard. The applicant files it at CESCO with three 2x2 photos.

When does a Puerto Rico driver need the DTOP-789 eye certificate?

For the heavy vehicle and Tipo 1-4 license categories. An ophthalmologist or optometrist authorized to practice in Puerto Rico completes Parts II and III, reporting acuity, depth perception and peripheral vision, and states whether the applicant can drive, with lenses or in daylight only. The applicant presents it at CESCO, and it is valid for six months. Commercial passenger license categories use the physician's DTOP-DIS-260a medical certification instead.

Who completes Part C of the SINOT SI-1 disability application?

Each treating physician, chiropractor or psychologist authorized to practice in Puerto Rico, or the medical records custodian of the treating institution. Each provider fills in a separate Part C certifying the diagnosis, treatment and period of non-occupational disability. The worker mails it with Parts A and B to DTRH, or delivers it to a DTRH office, within 3 months of the start of disability. There is no online filing. Workers covered by a private SINOT plan use that plan's own form.
