Puerto Rico DTOP-789 Eye Doctor Certificate (CESCO)

CESCO in Puerto Rico requires this Spanish-language eye exam certificate for heavy vehicle and Tipo 1 to 4 license categories. An ophthalmologist or optometrist records visual acuity with and without lenses, depth perception, nasal and temporal peripheral vision and Ishihara color testing, then certifies whether the applicant can drive and whether lenses or daytime-only driving are needed.

Who completes it

The applicant fills in Part I and signs; an ophthalmologist or optometrist authorized to practice in Puerto Rico completes Part II (Informe del Examen) in full and signs the Part III certificate with a license number.

Submitting and payment

How to submit it
The applicant presents it at CESCO, where the certificate is valid for six months.

Good to know

  • The form states Part II must be completed in all its parts: acuity, depth perception, peripheral vision, color appreciation and general findings.
  • Depth perception is graded as excellent, good, deficient or none, and peripheral vision is recorded nasally and temporally for each eye.
  • If the Ishihara plates are not normal, the examiner states whether the applicant can pass another comparable color test.
  • The examiner also answers whether the defect is congenital and whether the applicant should drive by day only. Related CESCO medical forms are DTOP-DIS-260 and DTOP-DIS-260a.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is the DTOP-789 valid?

Six months. The applicant presents the certificate at CESCO within that period, and the examiner records the date of examination in Part III next to their name, license number, address and phone.

Who can complete Puerto Rico's Certificado del Oftalmólogo u Optómetra?

An ophthalmologist or an optometrist duly authorized to practice in Puerto Rico. In Part III the examiner marks which they are, certifies having examined the applicant, and confirms the Part II results, adding license number, address and phone.

Is DTOP-789 available in English?

No. DTOP publishes it only in Spanish; Ophthalmologist or Optometrist Certificate is a translation. The key questions on it are whether the applicant must use glasses or contact lenses to drive and whether the examiner certifies the applicant can drive a motor vehicle.
