Puerto Rico DTOP-375: Seat Belt or Tint Exemption

Puerto Rico's CESCO requires this Spanish-language medical certification when a driver seeks an exemption from the seat belt law or permission for window tint darker than the law allows. A specialist physician states the patient's diagnosis, medications and doses, and how long the patient has been under their care, then certifies the exemption they recommend under Ley 22-2000.

Who completes it

A specialist physician authorized to practice medicine in Puerto Rico completes and signs the entire certification, including the patient's identifying details, with license number and contact information.

Submitting and payment

How to submit it
The patient presents it at CESCO together with DTOP-DIS-265, the Solicitud para Exención de Tintes y Cinturón.

Good to know

  • For tint, the physician certifies the condition requires more sun protection than Article 10.05 of Ley 22-2000 allows and recommends DTOP evaluate a percentage below the legal 35%.
  • For seat belts, the physician recommends exemption under Article 13.02 when the patient drives or rides as a passenger on public roads in a vehicle required to have belts.
  • The form exists only in Spanish. The physician checks one or both boxes at the top: Cinturón de seguridad, Tintes.
  • The diagnosis and medication lines can continue on an additional sheet if needed. CESCO's forms index lists it as DTOP-DISC-375.

Frequently asked questions

Is DTOP-375 available in English?

No. The Departamento de Transportación y Obras Públicas publishes it only in Spanish as Certificación Médica para Exención Cinturón de Seguridad / Tintes. The physician completes it in Spanish, marks the exemption sought and signs under 'Certifico correcto'.

Who can sign Puerto Rico's seat belt or tint medical certification?

A specialist physician (médico especialista) duly authorized to practice medicine in Puerto Rico. The physician names their specialty, states the period the patient has been under their care and gives license number, physical address, email and phone.

Does the physician set the tint percentage on DTOP-375?

Not exactly. The physician certifies that the condition requires darker tint than the law permits and recommends that DTOP evaluate allowing a percentage lower than the 35% permitted by law. The exemption request itself is filed on DTOP-DIS-265.
