Puerto Rico DTOP-DIS-260 Driver License Medical Form

Every applicant for a new learner's permit or driver's license in Puerto Rico needs this Spanish-language medical certification. Under Ley 22-2000, the physician personally examines the applicant, records visual acuity, ears, arms and legs and any history of epilepsy, seizures, loss of consciousness, heart disease, pacemaker or prosthesis, then certifies whether the person is physically and mentally fit to drive.

Who completes it

A physician licensed to practice in Puerto Rico completes everything after the applicant identification block and signs with license number and specialties; the applicant gives name, Social Security number and Puerto Rico license number and signs.

Submitting and payment

How to submit it
The applicant presents it at CESCO together with the license application.

Good to know

  • CESCO does not accept a certificate with blank spaces or alterations, so every item must be answered.
  • The physician must examine the applicant personally and write legibly in block letters, as the instructions to the physician require.
  • Acuity is recorded for each eye with and without corrective lenses, with whether the condition is congenital or acquired and whether the applicant wears glasses or contact lenses.
  • Applicants for commercial passenger categories use DTOP-DIS-260a instead, and DTOP-789 is the separate eye doctor certificate.
