Puerto Rico DTOP-DIS-010: Rótulo Removible Placard

Puerto Rico's DTOP uses this Spanish-only application for the rótulo removible, the removable disabled parking placard, whether new, renewed, duplicate, permanent or temporary. A specialist physician meets it on page 2, which certifies that the applicant has one or more conditions listed under Ley 22 of January 7, 2000, as amended. The applicant files it at a CESCO office.

Who completes it

The applicant, or a parent or legal guardian, completes page 1 and authorizes DTOP to request medical records or a DTOP-375b certification. A specialist physician licensed in Puerto Rico completes page 2, initials each qualifying condition, writes the diagnosis (ICD-10 codes accepted) and marks it permanent or temporary.

Submitting and payment

How to submit it
The applicant files the application in person at CESCO with three 2x2 photos, under Reglamento 6666.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is DTOP-DIS-010 available in English?

No. DTOP publishes the rótulo removible application only in Spanish, including the physician's certification on page 2 (Certificación de Elegibilidad por Médico Especialista). What conditions qualify for a rótulo removible in Puerto Rico?

The form lists 25, among them paralysis or amputation of the lower limbs, severe lung disease with FVC or FEV under 60%, chronic dialysis, cardiovascular grade III-C or higher, legal blindness, autism, severe Down syndrome, dwarfism, multiple sclerosis, moderate or severe hemophilia, lupus and skin cancers that hinder mobility.
