CNESST 5007-R: Rapport final, psychologie

This French-only CNESST report closes a psychological, psychotherapeutic or neuropsychological intervention for a worker with an employment injury. It analyzes the results for each objective, including the intrinsic and extrinsic factors that helped or hindered progress, and states why the intervention ended. If treatment stops before a progress report was due, CNESST requires only this final report.

Who completes it

The psychologist, psychotherapist or neuropsychologist who delivered the intervention completes and signs the whole two-page form. The diagnosis or reason for referral it restates comes from the worker's treating health professional, who also receives a copy.

Submitting and payment

How to submit it
Send it to CNESST within 15 days of the last meeting, through the online document-sending service (Envoi de document) or by fax, with a copy to the worker's treating health professional.
Who pays
CNESST pays up to two hours of report-writing time at the regulated hourly rate, billed on the Compte de soins ou de services de psychologie ou de psychothérapie (5008), and the worker cannot be charged.

Good to know

  • Meeting dates are listed only for the period since the last report sent to CNESST, so earlier sessions stay documented in the progress reports (5006-R).
  • As with the progress report, attach the 'Informations communes aux rapports' page from the Compte de soins ou de services de psychologie ou de psychothérapie (5008).
  • The report restates the problem linked to the employment injury that was identified at the initial evaluation, then sets out the reasons the intervention ended.
  • Providers must be members in good standing of the Ordre des psychologues du Québec or hold its psychotherapist permit, and both provider and clinic must be registered CNESST suppliers.

Frequently asked questions

When is the CNESST final psychology report required?

When the psychological, psychotherapeutic or neuropsychological intervention ends. CNESST must receive it within 15 days of the last meeting, with a copy going to the treating health professional. If the intervention ends before a progress report (5006-R) was due, CNESST requires only the final report, not a progress report.

How much report-writing time does CNESST pay for the 5007-R?

Up to two hours of report-writing time is payable at the regulated hourly rate, compared with up to one hour for a progress report. Bill it on the Compte de soins ou de services de psychologie ou de psychothérapie (5008), which serves as the invoice. You cannot claim any amount from the worker for services covered by CNESST.

What does the final report ask you to analyze?

For each objective, it asks for an analysis and evaluation of results against progress indicators, including the intrinsic and extrinsic factors that contributed to or hindered reaching it. It also records the therapeutic activities used, the worker's perception of whether each objective was met, the reasons the intervention ended and any additional notes.
