CNESST 5006-R: Rapport d'évolution, psychologie

Psychologists, psychotherapists and neuropsychologists treating a worker with an employment injury on the treating health professional's prescription use this French-only CNESST report to document progress. It is due for each block of 6 to 12 hours of intervention, or every 3 months, whichever comes first. It reviews each objective against progress indicators, records the worker's own view of their progress and flags changes to the individualized intervention plan.

Who completes it

The psychologist, psychotherapist or neuropsychologist delivering the intervention completes and signs the whole two-page form. It restates the diagnosis or reason for referral given by the worker's treating health professional, who also receives a copy.

Submitting and payment

How to submit it
Send it to CNESST within 15 days of the last meeting the report covers, through CNESST's online document-sending service (Envoi de document) or by fax, and send a copy to the treating health professional.
Who pays
CNESST pays at the hourly rate set by regulation, with up to one hour of report-writing time payable when billed on the Compte de soins ou de services de psychologie ou de psychothérapie (5008), and the worker cannot be charged.

Good to know

  • A progress report is still due once 3 months have passed even if fewer than 6 hours were delivered; if treatment ends before one is due, send only the final report (5007-R).
  • The form reminds you to attach the 'Informations communes aux rapports' page, which is part of the Compte de soins ou de services de psychologie ou de psychothérapie (5008).
  • Intervention begins only after the worker gives the prescription to CNESST and CNESST sends you a Demande de soins; you and your clinic must be registered CNESST suppliers.
  • Page 2 asks for the number and frequency of meetings still planned, plus any changes to the individualized intervention plan and your recommendations.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How often is the CNESST psychology progress report due?

For each period of 6 to 12 hours of intervention, or every 3 months of intervention, whichever comes first. If 3 months pass with fewer than 6 hours delivered, a report is still required. The form lists the dates of up to 12 meetings for the period, and CNESST must receive it within 15 days of the last of those meetings.

Is the 5006-R available in English?

No. CNESST publishes the 5006-R in French only, as a two-page fillable PDF dated 2025-12. Page 1 identifies the worker by name and CNESST file number, with the date of the original event and of any recurrence, relapse or aggravation, then lists the period of care, the meeting dates, the objectives and the therapeutic activities used.

Is writing the progress report paid?

Yes. Up to one hour of report-writing time is payable at the regulated hourly rate, billed on the Compte de soins ou de services de psychologie ou de psychothérapie (5008). CNESST states that you must complete the evaluation report and, where applicable, the progress and final reports to be paid, and you cannot claim any amount from the worker.
