CNESST Form 1845: Psychosocial Progress or Final Report

Professionals providing psychosocial intervention services at CNESST's request use this five-page report to update CNESST on the worker's progress or to close the psychosocial intervention plan. One form serves both purposes: tick progress report or final report. It revisits the goals and progress indicators set at evaluation, the activities delivered, the worker's progress and perception, results, any plan changes and the reasons for ending.

Who completes it

The service provider completes and signs the whole report, choosing a specialty such as psychologist, neuropsychologist, psychotherapist, social worker or psychoeducator and entering their CNESST-authorized supplier number.

Submitting and payment

How to submit it
Send it to CNESST through its Envoi de document online service or by fax, whenever you report on the worker's progress and at the end of the psychosocial intervention plan.
Who pays
CNESST pays for the services at the rate set in the professional-services contract under which it requested them.
