CNESST Form 1844: Psychosocial Evaluation Report

Professionals who provide psychosocial intervention services at CNESST's request complete this five-page report after evaluating the worker's psychosocial needs. The services address personal difficulties arising from the employment injury that hinder social or occupational rehabilitation. The report covers the reasons for referral, case history, symptoms, the worker's perception of their situation and return to work, conclusions, and any recommended intervention plan.

Who completes it

The service provider, identified by specialty and CNESST-authorized supplier number, completes all eleven sections and signs with their title; section 8 applies only to a neuropsychological evaluation.

Submitting and payment

How to submit it
Send the completed report to CNESST through its Envoi de document online service or by fax.
Who pays
CNESST pays for the evaluation at the rate set in the professional-services contract under which it requested the services.

Good to know

  • This form is only for psychosocial services CNESST requests in rehabilitation; psychological evaluation following a treating professional's prescription uses a different report, form 5005-R.
  • Section 6 lists symptoms the worker describes: trauma, cognitive problems, anxiety, anger or irritability, suicidal ideation, depression, substance use, isolation or social withdrawal, and others.
  • A recommended plan in section 10 needs goals agreed with the worker, a progress indicator for each goal, expected participation, prognosis, start date and number and frequency of sessions.
  • Record up to four evaluation meeting dates in section 4, and use the annex on page 5, naming each section, when you need more space.
