West Virginia OIC-WC-3OP: Pneumoconiosis Report

Physicians use this three-page West Virginia report to give their opinion on whether a worker has occupational pneumoconiosis, how long the disease has been present and whether it impairs capacity for work. It belongs to an occupational pneumoconiosis claim alongside the employee's OIC-WC-1OP and the employer's OIC-WC-2OP, and includes a NIOSH chest radiograph classification form. The carrier, self-insured employer or TPA may then refer the claim to the Occupational Pneumoconiosis Board.

Who completes it

The examining or treating physician completes the whole report, covering history, complaints, sputum testing, X-ray and pulmonary function studies and physical findings, then signs it. The attached chest radiograph interpretation is completed by the film reader, who classifies film quality and parenchymal and pleural abnormalities.

Submitting and payment

How to submit it
Return the signed report and radiograph classification to the private carrier, self-insured employer or TPA administering the claim. The PDF is fillable, with buttons to save and print the completed form.
Who pays
No fee is printed on the form; medical services on West Virginia claims follow the OIC's maximum allowed reimbursement schedule (West Virginia Medicare rates plus 35%), though private carriers, self-insured employers and approved managed care plans may set their own fees.

Good to know

  • The history section asks about pneumonia, pleurisy, asthma, tuberculosis, arthritis and heart disease such as angina, coronary occlusion and congestive heart failure, with dates, plus other serious illness, surgery and accidents.
  • List every chest X-ray, pulmonary function study and blood gas study with the facility or doctor, date, where it is filed and findings; the report also asks about OP treatment programs.
  • The radiograph sheet is the CDC/NIOSH Roentgenographic Interpretation form, recording ILO profusion, opacity shape and size, affected zones, pleural plaques, diffuse thickening and other abnormalities.
  • The OP Board, acting under W.Va. Code 23-4-8a, examines questions of exposure and the degree of pulmonary function impairment on claims the carrier, self-insured employer or TPA refers.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does Form OIC-WC-3OP include the ILO X-ray form?

Yes. The OIC lists the report as including the ILO form, and the PDF carries the CDC/NIOSH Roentgenographic Interpretation sheet. The film reader records film quality, parenchymal abnormalities consistent with pneumoconiosis by shape, size, zone and profusion, pleural findings, other abnormalities, and whether the worker should see a personal physician.

What opinion does the physician give on the West Virginia pneumoconiosis report?

Page one asks whether, in your opinion, the claimant has contracted occupational pneumoconiosis, how long they have had it, and whether it has impaired their capacity for work and to what extent. Page two records physical findings: appearance, height, weight now and a year ago, breath sounds, heart sounds, pulse and blood pressure.

What role does the Occupational Pneumoconiosis Board play?

The carrier, self-insured employer or TPA administers the claim and may refer it to the Occupational Pneumoconiosis Board. The Board conducts examinations on questions of exposure and the degree of pulmonary function impairment, while the OIC's Claims Services schedules the Board's docket, tracks exam requests and checks that required information is included.
