West Virginia Workers' Comp Controlled Substance Form

West Virginia workers' compensation claim administrators use this questionnaire for physicians prescribing opioids on a claim. It asks for the diagnosis and body part responsible for the pain, pain type, substance abuse and psychological history, unrelated conditions, a detailed pain history, prior medications and therapies with results, tapering attempts, the objective findings supporting continued opioids, consultations and random testing.

Who completes it

The prescribing or treating physician completes and signs the whole form, adding vendor and physician numbers and the physician's name and address.

Submitting and payment

How to submit it
Return it to the private carrier, self-insured employer or third-party administrator handling the claim, enclosing a copy of any signed statement from the claimant on long-term opioid risks.
Who pays
The OIC form does not set a fee for completing it or say who pays the physician.

Good to know

  • By signing, the physician acknowledges contractual obligations to the claim administrator and agrees to release office notes and test results immediately.
  • The certification cites W. Va. Code §61-3-24G, which carries penalties for knowingly certifying a false report or withholding material facts.
  • Record pain levels before and after analgesia, and whether the claimant's pain interferes with activities of daily living, describing the limitations.
  • Rate the claimant's potential to return to the pre-injury employment position as excellent, good, fair or poor.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

What does West Virginia's Controlled Substance Form ask about opioid tapering?

Question 11 asks whether you have attempted to decrease the opioid dose and, if so, when and at what intervals; if not, why not. Question 12 then asks which objective findings support the need for continued opioid therapy.

Do I need a signed patient statement for long-term opioids?

The form asks whether you discussed the risks and side effects of long-term opioid therapy with the claimant and whether you have a signed statement showing their understanding. If you have one, enclose a copy with the completed form when you return it to the claim administrator.
