West Virginia workers' compensation administrators accept this one-page form from the physician of record to update the diagnoses on a claim. Who completes it

The physician of record completes the claimant and physician information, including the physician's FEIN, and signs and dates it.

Submitting and payment

How to submit it
Mail it to the private carrier, self-insured employer or third-party administrator administering the claim, as the form's instructions direct.
Who pays
The OIC form does not set a fee for completing it or say who pays the physician.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
