West Virginia Attending Physician Benefits Form (WC)

West Virginia workers' compensation claim administrators use this one-page form to support temporary total disability (TTD) payments. The attending physician estimates the period of disability, the transitional or full-duty return-to-work date, whether the claimant has reached maximum medical improvement and whether they are ready for a permanent partial disability rating, along with current and next appointment dates.

Who completes it

The attending physician completes and signs the top section. The claimant completes the bottom, certifying whether they worked or received income while certified temporarily and totally disabled.

Submitting and payment

How to submit it
Return it to the entity administering the claim: the private carrier, the self-insured employer or the third-party administrator.
Who pays
The OIC form does not set a fee for completing it or say who pays the physician.

Good to know

  • Physicians must also send updated, detailed medical reports with current treatment plans after each office visit, because the form alone does not guarantee that TTD benefits will be paid.
  • Failure to complete the form in its entirety affects payment of TTD benefits, so fill in every field, including MMI and readiness for a rating.
  • The OIC lists it among third-party administrator forms used on OIC-administered claims, posted as an example of an approved form.
  • New or changed diagnoses on the claim go on the separate Diagnosis Update form completed by the physician of record.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Why was a TTD payment delayed after I sent the Attending Physician Benefits Form?

The form warns that failing to complete it in full affects TTD payment, and that completing it without updated detailed medical reports, including current treatment plans, after each visit does not guarantee benefits. Check that both sections were filled out and that office notes reached the administrator.

Does the claimant sign the West Virginia Attending Physician Benefits Form?

Yes. Below the physician's section, the claimant answers whether they performed any work or received income for work while certified temporarily and totally disabled, and signs a certification that the answer is true, acknowledging penalties for fraudulent statements.
