West Virginia Claim Reopening Application for PPD

An injured worker in West Virginia uses this application to ask that a claim be reopened for a new permanent partial disability rating. It reaches the clinician after the worker completes Section I, citing either aggravation or progression of the compensable condition or factors the OIC did not consider before. The physician's findings and rating then go, with any supporting evidence, to the carrier, self-insured employer or TPA administering the claim.

Who completes it

The claimant completes Section I (grounds for reopening, later injuries or claims, unemployment or wage replacement benefits, current employment and retirement) and gives the form to the doctor. The treating physician, or a new treating physician, completes Section II in detail and attaches a narrative report if needed.

Submitting and payment

How to submit it
Once Section II is signed, the application goes with the narrative report and any supporting evidence to the private carrier, self-insured employer or third-party administrator handling the claim.
Who pays
No fee is printed on the form; medical services on West Virginia claims follow the OIC's maximum allowed reimbursement schedule (West Virginia Medicare rates plus 35%), though private carriers, self-insured employers and approved managed care plans may set their own fees.

Good to know

  • Section II asks whether the condition has aggravated or progressed since the worker was released to work or found at maximum medical improvement, with a short narrative of the physical findings.
  • The rating must use the Range-of-Motion model, apply Title 85, Rule 20, and include the tables and charts behind the whole person percentage.
  • List any earlier percentages awarded for the same body part and state whether they should be subtracted from the new impairment rating.
  • Give the current diagnosis with ICD-10-CM codes, and say whether you are requesting a rating on a different body part than the original claim covered.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What grounds does West Virginia accept for reopening a PPD claim?

The claimant checks one of two grounds in Section I: aggravation or progression of the condition or disability from the compensable injury or occupational disease, or facts about the disability that the OIC did not consider in earlier findings. The physician's Section II then documents the physical findings behind any aggravation or progression in a short narrative.

Can a new doctor complete the reopening application?

Yes. Section II asks whether you were the treating physician for the claim or are a new treating physician, and the date of the examination your findings are based on. The physician also states whether the worker can return to the regular job or light duty, would benefit from rehabilitation, and has reached maximum medical improvement.

Which impairment method does the reopening form require?

Question 10 asks for the Range-of-Motion model, with the tables and charts used to reach the whole person percentage, applying Title 85, Rule 20 to the exam. Question 11 then asks for any prior percentages given for the same body part and whether they should be subtracted from the new rating.
