Nebraska FH-43 Certification of Physical Examination

Instead of a full exam report, a Nebraska examiner can certify on this short form that a student received the complete physical examination required for entry at the beginner grade, seventh grade or an out-of-state transfer (Neb. Rev. Stat. 79-214). It can also carry the visual evaluation results, and the parent consents to release it to the school.

Who completes it

The qualified medical examiner, a physician, physician assistant or APRN, completes Part I with the exam date, signature and printed name. The parent or guardian completes Part II, consenting to release to the named school.

Submitting and payment

How to submit it
The completed certificate is returned to the school health office, with the examiner's address or clinic stamp.
Who pays
The form sets no fee and does not address payment for the physical examination.

Good to know

  • The form quotes the Nebraska Administrative Code: a printed or typed form signed by a physician, PA or NP showing an exam on a specific date within the previous six months is sufficient evidence.
  • The examiner marks whether a visual evaluation was completed and, if so, reports pass, fail or further evaluation for amblyopia, strabismus, eye health and acuity.
  • Acuity is recorded at 20 feet and 16 inches for each eye, with or without glasses, in the same layout as FH-41.
  • FH-43 is the short alternative to the full FH-42 report; the current version is dated 7/2024, with a Spanish edition available.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is the difference between Nebraska FH-42 and FH-43?

FH-42 is the full physical examination report with findings, hearing screening, medications and an athletics certification. FH-43 is a short certificate in which the examiner attests that the required complete physical was done on a stated date, with optional visual evaluation results.

How recent must the exam be on Nebraska FH-43?

The form quotes Nebraska Administrative Code Title 173: a signed form showing a physical examination was administered on a specific date within the previous six-month period, on a named individual, is sufficient evidence of an exam by a qualified examiner.

Who can sign the Nebraska Certification of Physical Examination?

Part I is signed by the qualified medical examiner, which the form defines as a physician, physician assistant or advanced practice registered nurse. The examiner prints their name and adds an address or clinic stamp.
